How to reset the phone to factory default

I manually worked out the following steps on a phone with a broken screen by going through the same steps on the replacement phone (except the very last one):

1. open the phone
2. ok
3. 9
4. 6
5. 6
6. xxxx (4 digit PIN, last 4 digits of your phone number by default)
7. 6
8. ok
9. xxxx (4 digit PIN, last 4 digits of your phone number by default)

The phone will then reboot and come up in a factory default mode.
